At Avicenna Centre for Brain Health, we offer legal services and other support documentation as required by government or insurance agencies when necessary.

We Offer Independent Medical Evaluation for the following purposes:

• Independent medical evaluation
• Capability assessment
• POA
• Committeeship
• Representative agreement
• Medical Legal Consultations requested by lawyers and WCB for conditions involving mental health concerns

Please note, all these services are private pay and are not covered by MSP. As a result, no family physician referral is required. For fees regarding any of these services please contact the clinic directly and an invoice will be arranged.
